Cleaned up the comment style and added authors for the TODOs in these files.

(Most of the TODOs are really old at this point. So I'm not sure if they are helpful anymore in some cases?)

Cleanups like this (just getting code to adhere to our current style guidelines) can just be commited directly, IMO they don't have to be reviewed.
