The normals of the cage mesh are pointing inside instead of outside, they need to point in the right direction to determined the inside/outside.
I've added a note about this to the documentation now.

Thank you! Other guys from blenderartist have come to the same issue (http://blenderartists.org/forum/showthread.php?298128-Mesh-Deform-Modifier-not-working) and I didn't see the last comment which solved the problem.
